Getting Started

I’m interested! What are the next steps?

Please contact us to discuss this by phone or schedule a video chat. We’d love to get to know you better, answer any questions you might have, and have the opportunity to introduce you to a few members of our team!

What articles of clothing should we bring?

Pack about a week’s worth of comfortable, everyday clothing, including pajamas and undergarments. Comfortable, closed-toe shoes with a good sole matter more than most people expect, because you’ll be up and walking in therapy. Loose, easy-on clothing makes dressing practice easier. Label everything if you can. If you arrive without enough, tell us and we’ll help you sort it out rather than have you go without.

What type of clothing items should I bring?

We encourage you to bring a pair of comfortable shoes and at least seven days worth of comfortable clothing including pajamas and undergarments.

Can I bring personal belongings?

Yes! We encourage you to bring personal items such as family photos, blankets, and decorative items. Anything that will help make your stay more comfortable.

Insurance & Billing

What insurances do you accept?

We accept Medicare, Medi-Cal, private pay, and a wide range of HMO and PPO plans. Because coverage varies so much from plan to plan, the most useful thing we can do is check yours directly. Give our team your insurance information and we’ll verify your benefits before admission, then walk you through what’s covered, what isn’t, and what you’d be responsible for. No guesswork, and no surprises after you arrive.

How do I pay my bill?

To pay your bill, visit our Online Bill Pay page or give us a call.

Healthcare Services

Do you offer short-term rehabilitation?

Yes. At Pleasant Hill Post Acute, your successful recovery and quick return home are our top priorities. We provide a unique personalized experience whether you’re suffering from an illness or recovering from a major medical event. Physical, occupational, and speech therapy is conducted under the guidance of licensed experts and is available seven days a week.

When will the doctor see me?

A physician typically sees you within 24 to 48 hours of admission. In the meantime, our nursing team is in contact with your physician from the day you arrive, so your orders, medications and care plan are in place immediately rather than waiting on that first visit. If anything changes before then, we reach out to your doctor right away. You’ll never be waiting on a visit for something that needs attention now.

How often will I have therapy?

Most patients begin therapy within 24 hours of admission and receive it up to five days a week, though your exact schedule depends on your condition and what your physician orders. Our therapy team is here seven days a week. We review your progress weekly and adjust as you improve, so the intensity follows your recovery instead of a fixed template.

How long is a typical stay?

It depends on why you’re here and how your recovery goes, so there’s no single answer we could give honestly. What we can tell you is how it’s decided: your team reviews your progress every week, measuring where you are against the independence you’ll need at home. We’ll talk with you and your family about the likely timeline early, and keep you updated as it becomes clearer. You won’t be guessing about your own discharge date.

Do I need to bring my medications?

No. We order your medications and store them securely here, so you don’t need to bring anything from home. What we do need is a complete and accurate list of everything you take, including over-the-counter medications, vitamins and supplements. Bring that list, or bring the bottles for us to record, and our nursing team will handle the rest from there.

How do you protect patients, staff, and visitors from COVID-19 and other infectious diseases?

Our team is highly trained and experienced in caring for high-acuity patients and infectious diseases like COVID-19. Our dedicated infection prevention nurse helps ensure we follow current infection prevention guidelines and best practices. This includes educating all staff about the appropriate use of personal protective equipment (PPE) such as N95 masks, surgical masks, gloves, and gowns and when to use COVID-19 protocols required by local department of public health and/or the CDC.

Questions & Concerns

Are activities available?

Yes, every day. An activity calendar is posted in every room, so you and your family always know what’s scheduled and can plan visits around it. Nothing is required and no one will push you. Tell our activities director what you actually enjoy and they’ll steer you toward the things you’ll look forward to rather than the things you’d politely sit through.

What is your visitor policy?

Visiting hours are 8 am to 8 pm, seven days a week. We encourage family and friends to visit, because familiar faces do real work in recovery. If you need to visit outside those hours, talk to us. We understand that work schedules and travel don’t always cooperate, and we’ll do what we can to accommodate you.
