My mom needed post surgery care, and I passed on the initial care facility assigned – because I got a really good vibe from Pleasant Hill Post Acute… and my gut was right. This facility is fantastic! The staff, the care, the patience with the patients… LOL. I have only positive things to say about the whole 2 week experience. My mom is prepping for her transition home – and I am so grateful to this entire team.
I want to express my gratitude for the excellent care at Pleasant Hill Post-Acute Care. I was admitted with a fractured pelvis and could not walk. Thanks to the entire staff’s exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but here are a few. I’d like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients’ well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient’s needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfast always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs.
I was all prepared when my husband was discharged over a month ago,to give a nice review on how nice the staff was etc. that’s my only nice comment. My husband went in for phys therapy after breaking a hip. Therapy was questionable to say the least , I was there 2-3 times everyday ,they put on a good front. After he came home. We discovered horrible sores on his feet and rear ,which I was told everyday he was getting wound care for. , conveniently they had socks on him everyday. So I did not see them till he got home. His legs were atrophed from no useful physical therapy ,he came home unable to walk at all. ,bed ridden and with sores that turned into the most horrible painful sores I have ever seen ,even after 2 month of 3 days a week of wound care ,they had gotten too far in the rehab to heal. He died last week partially from these sores that eventually got infected and went to his bones. My advice ,before you get an okay and acceptance from a care home to take your loved one in. Make sure they are absolutely equipped and capable of the care this person needs. This place was not the right place for my husband.

Pleasant Hill Post Acute was an amazing rehabilitation program, and I’m so glad I chose it. I really appreciate everything they did to help me recover and regain my mobility after surgery. I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. I especially want to thank Marth and Arryan–they are so sweet and genuinely care about their patients. The night nurses were excellent as well. I’d also like to thank the CNAs Melissa, Beth, and of course Dalia. They were helpful, kind, and attentive. They took such great care of me and made me feel safe during my time away from home. Physical therapy and occupational therapy were also great throughout my recovery. They helped me from the very beginning to regain my mobility and strength. The therapists Jordan and Justin were fun, funny, and kind throughout my sessions. They made each visit engaging and always took the time to listen to my needs. They consistently found ways to challenge me while keeping the sessions enjoyable, and they helped me grow stronger every day. I truly appreciated their support and encouragement. My case manager, Bianca, was amazing throughout the whole process. She was always available whenever I had questions or needed someone to talk to. We definitely had our moments–lol–but she was truly supportive and helpful every step of the way. I also want to thank Claire, the activity director. She was so lovely and caring, and she truly tried to make every day fun and different for all the patients. She recognized each patient’s unique qualities and worked hard to make their time at the facility as enjoyable as possible. Thank you to everyone at Pleasant Hill Post Acute for helping me through my recovery journey.
The staff is very helpful when you need #1 replaced, taken care off, and your personal needs. Thank you Olivia, Justin, Jordan, Martha, Bo.
My stay here has been really great. Everyone has been very friendly and helpful especially Jordan and Nikki in therapy they are wonderful people to be around. Nurses Olivia and Martha were also amazing and Claire and activities helped make everything fun. Overall it has been a great experience and I really enjoyed my time here. I would definitely recommend this place to others.
Olivia was the best CNA I ever had. Martha was the best nurse I ever had. Thanks Jordan and Justin the best therapy. Julie another great CNA. Thank you to the kitchen staff the food was excellent, the burger is delicious. If it was up to me I’d stay here another 3 months. The building and room service is the best. The place is so clean thank you housekeeping.
Awesome. Very attentive, professional. Moms second stay. We would recommend it without reservation. Clean. They really helped with her rehab.
They have an excellent staff care. This is gonna be the third time that my care client was admitted here at Pleasant Hill Post Acute. All the staff are very pleasant from the management, nurses, cna house keeping and maintenance. Service are way above and beyond. The atmosphere here are excellent you don’t feel isolated. I highly recommend this facility.
My wife was in this rehab facility for one week after a week long hospital stay. Excellent care by the entire staff to make her feel comfortable and at home. Excellent physical therapists Niki and D’Andre did a great job working with her and making her feel like part of the family. This is a very loving, caring facility. Beau and all of the staff are very kind and understanding and get excellent care. My sister was here for a month and really became a part of the community & healed
My uncle has been here and it has been a wonderful experience. We truly appreciate the nurses, Martha and Viki, for their excellent care and compassion. Thank you as well to Olivia, Beth, and the other staff members whose names I may not remember but who were equally amazing. A special thank you to Geraldine at the front desk for always being so welcoming and kind. Lastly, we are very grateful to Bianca, the case worker, for making the discharge process smooth and stress-free. We highly recommend this place for your loved one.
My parents have been to several rehab facilities. This one had the best food and staff.
We are very thankful for the care my sister received during her stay. The facility is clean, organized, and has a friendly environment. I cannot remember everyone’s name but thank you to Bianca (case manager) for always following up. Appreciate all of the staff.
Pleasant Hill Post Acute is an exceptional rehabilitation facility. The staff members are universally friendly, polite and cheerful to daily visitors like me as well as the patients. My wife recently spent a little over six weeks at PHPA, first awaiting surgery for, then rehabbing a severely broken ankle. She mentions her nurses (Martha, Jackie, Bo and especially Amanda), CNAs (especially Victoria), physical therapist Jordan, and occupational therapist Sara as all being compassionate, skilled and friendly practitioners of their professions. The administrative staff adroitly handled all my questions and concerns (I’m a bit of a helicopter husband in situations like this), and were never less than understanding, competent and extremely helpful. Marilyn made the exit process smooth and simple, and supported the flexibility we needed. All in all, given that we didn’t really want to be in a skilled nursing facility at all (who chooses to be injured?), PHPA couldn’t have been better. I heartily recommend Pleasant Hill Post Acute to anyone needing its services.
Both of my parents were in this rehabilitation center, and we are very satisfied. Excellent care, attentive staff, and good nutrition. Special thanks to the physical therapists for their professionalism and dedication — sometimes working with my parents twice a day and even on Sundays. Thank you to the entire staff for the great care and support.
I have been to 3 facilities. I found this one to be the cleanest, the most helpful, and genuinely always willing to make you comfortable. Very comfortable. Loved it !!!.

Wonderful place, they took such great care of my Mom when she was recovering from surgery. We were able to visit her here and have lunch on the back patio in the sun. The doctor was like an angel. He was so encouraging in her recovery. And the head nurse amazing in his diligence to care for her wounds.

The facility is so outstanding! The workers have been trained in how to care with patients like they have been doing this their whole life! I would 100000% recommend this facility to anyone needing to go to a skilled nursing facility! They surly know what they are doing! The facility also has a super nice outside patio! I cannot express in words how excellent this facility is but what I do know is that it is definitely over and above what you expect!
